WHAT YOU NEED TO DO BEFORE THE EXAM

Login to https://medxpress.faa.gov and complete your application.

Print out or keep a record of the Confirmation Number. This will be in an e-mail to you and also at the bottom of your copy of the completed application form.

If you can, print a copy of the completed form. Make sure that the print setting on your browser is set to letter and the Fit to Page box is checked. The copy is not required, but saves time.
WHAT YOU NEED TO BRING WITH YOU TO THE EXAM

Goverrnment ID such as a driver's license or passport is required.

Remember the Confirmation Number.

Bring your glasses - both for distance and reading. (Contacts are OK.) You will have to pass both a near and distant vision test.

Bring copies of any FAA Special Issuance letters, Waivers, or Color Vision Letters.

If you are being treated for any chronic medical conditions such as hypertension, hypothyroidism, prostate cancer, or migraine headaches, bring a Current Status Report from your treating physician. This should be in the form of a typed letter on letterhead signed by the physician or an electronically signed electronic medical record printout. The content of the letter will of course vary with the condition being reported on. For example, a report on hypertension should state that the patient's blood pressure is under control. The blood pressure readings should be included. If on diuretic, then current (within 3 months) laboratory reports should also be included. The letter must list all medications and state that there are no side effects from the medications.
